I hereby register myself with the Organic Production Group __________________________________

as an organic farmer and agree to participate in the participatory guarantee system of the Caritas Da Lat farmer network (CFGS).

I COMMITTED TO COMPLYING WITH ORGANIC STANDARDS IN AGRICULTURE TO SERVE SOIL HEALTH, ENVIRONMENT, FAMILY AND COMMUNITY. THEREFOR, I PROMISE TO DO THE FOLLOWING:

 1. I will strictly follow the organic production standards set forth by the CFGS;

2. I will jointly develop regulations of the Production Group and comply with these regulations;

3. I will work cooperatively with other team members and participate fully in meetings and training sessions required by the Production Team and CFGS to share and expand my knowledge of organization and organic production techniques;

4. I consent/agree to the members of the Production Team to inspect all farms as well as the processing places, the storage places for raw materials, tools, machines, and products related to the organic farm and my housing if necessary without prior notice;

5. I will participate in the inspection of other farms as prescribed by the group;

6. I will keep good record books, update details about the whole farm and production activities including: origin cost of input materials, harvest output, chemicals invoices and sales records. I agree to have members of the Production team inspect my documents and records.

7. I will check with the Production team the input materials that I do not know well before putting these materials to use in the production area

8. When there is any change in production including the change of crops and the field management plan, I will immediately notify the Production Team leader immediately.

9. I will only ship products and use the CFGS trademark when the products are produced in certified organic or certified organic areas.

10. I will report to the Production Team if I have any minor or unintentional violations while implementing CFGS organic standards at my production place.

11. I accept the decisions of the Production Team in relation to my certification; my consent/agreement is final.

I have read the documents and have a good understanding of PGS including organic standards.

I hereby declare that the last chemical that were used on my farm was on ……………………. After that I have completely stopped using chemicals.

I certify that all information provided by me in the application and during the appraisal of my farm is true and correct, and will make sure to continue providing the information as soon as there is any change in my farm.
